MJHS Hospice and Palliative Care seeks a Field Case Manager (RN) to visit patients in community and facility settings, perform physical and psychosocial assessments, and electronically document visit occurrences. You will develop and implement nursing plans of care and make recommendations based on patient needs. The schedule includes one weekend per month and 2-3 on-call shifts monthly, with car provided for on-call. #J-18808-Ljbffr MJHS
