Job Description:

The RN Case Manager will provide acute inpatient case management and discharge planning experience in a large hospital setting, working closely with the social work team and other departments to manage patient care efficiently.

Responsibilities:

Perform acute inpatient case management for 15-20 patients per day.
Work with a team of social workers to coordinate patient discharges and care.
Cover all inpatient units throughout the hospital.

Requirements:

Minimum 3 years of Acute Inpatient Case Management experience, including at least 1 year in a large hospital (500+ beds, Level 1/Academic Medical Center).
Previous travel experience in a Level 1 facility.
Strong computer skills with Epic EMR experience.
Nursing degree required; prefer RN, BSN, but would consider ADN.
BLS certification through AHA required; CCM preferred but not required.
Recent (within the last two years) acute inpatient case management experience.
