Job Description Summary:

Position Responsibilities

• Interprets orders and verifies orders.
• Accurately compounds and dispenses pharmaceuticals.
• Monitors drug therapy regimens.
• Provides clinical consultation and clarification to practitioners and recommends appropriate alternative.
• Accurately and completely documents all clinical activities, drug regimen review and interventions
• Supports the formulary, rational drug therapy selection.
• Provides accurate and timely medication information.
• Assists with clinical services.
• Effectively and proactively follows-up with medical staff on any issues regarding drug therapy.
• Supports and participates in the goals of the organization's quality improvement, risk management and patient safety programs to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
• Documents medication errors and adverse drug reactions in the incident reporting system and notifies manager.
• Participates in the training of new pharmacists, pharmacy students and technicians.
• Maintains professional competency sufficient to meet current and future needs of the facility.
• Aware of the patient population served and caters to their health care needs
• Provides basic technical support for automated dispensing machines and other pharmacy computer systems.
• Effectively supervises pharmacy operation activities and provides support to the whole pharmacy team
• Compliant with facility pharmacy schedule and Oncall to work physically at patient care areas to interact with medical staff, nurses, and other members of the healthcare team and patients.
• Trains and participates with Disaster Team drills and real-event response as assigned
• Practices and adheres to the "Code of Conduct" philosophy and "Mission and Value Statement"
• Maintains appropriate controlled substance records.
• Complies with all regulatory standards (USP 797/800, Florida Board of Pharmacy, TJC, and others as applicable)
• Prioritizes work to comply with facility medication turn-around time policy and procedure.
• Effectively supervises and assists pharmacy technicians and other support staff
• Acts as supervisor in charge as assigned.
• Participates in the continuous quality review of order sets and/or computer drug entries.
• Participates in the continuous quality improvement of pharmacy operations (operational/clinical)
• Effectively manages medication waste billing per regulatory standards and policy.
• Maintains outpatient pump documentation, inventory and records per Infusystem requirements.
• Trains and maintains appropriate USP 797/800 records in the Sterile Compounding documentation system used by the institution and technician oversight of documentation
• Follow applicable REMS program requirements
• Complies with ASCO QOPI standards and 340B program requirements
• Ensures drug procurement, biosimilar subsitutions, and drug shortage mitigation planning

Qualifications

• License/Registration/Certification
  • Registered Pharmacist in the State of Florida.
• Education
  • B.S. Pharmacy Degree from an ACPE accredited School of Pharmacy.
• Experience
  • 2 years oncology pharmacy experience preferred.
