Candidate must be a clinically competent registered nurse. The CCM nurse works with patients to educate them on chronic disease management in the patient’s home. This position will also have a special focus on diabetes education. Candidate counsels the patient and family in meeting nursing and related needs, participates in in‑service programs, supervises and teaches other nursing personnel. Graduate of an accredited school of nursing currently licensed in the State of Ohio. Responsibilities Provides education to patients/family on chronic disease processes Provides direct consultation using in-depth clinical knowledge and skills in area of Diabetes to groups and individual patients with Type 1, Type 2, Gestational Diabetes and Pre Diabetes patient Assists in Diabetes Self‑Management Education and Support (DSMES) program as well as Diabetes Prevention Program (DPP) Receives and assesses referrals from referring providers. Consults with multidisciplinary team to develop, implement and evaluate care for patients and families. Documents patient care activities in patient medical records Complete documentation accurately and timely. Coordinate patient care with appropriate disciplines. Physician consultation for changes in patients' condition. Requirements Registered nurse with two years of experience in delivering nursing care Current CDCES/CDE or willingness to obtain certification Analytical abilities and sound nursing knowledge. Knowledge of policies and procedures. CPR certification. Reliable transportation, current driver's license, and automobile insurance. LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
