About: About CRC: Our mission is to help our neighbors create paths to healthy food, stable homes and safe relationships. We do this through a variety of programs including emergency assistance, domestic violence emergency shelter, counseling, housing stability and access to food. Community Resource Center is widely recognized as the primary provider of social services and domestic violence programs in coastal North County San Diego, serving more than 7,200 people each year.

Position Profile: Under the direct supervision of the Social Services Program Manager, the Social Services Case Manager provides housing-focused, participant-centered case management and supportive services to individuals and families enrolled in CRC programs or those seeking services and resources. The Case Manager supports participants in identifying and addressing barriers to housing stability, increasing self-sufficiency, and working toward individualized goals.

 

Services include case management, crisis intervention, emergency assistance, housing navigation and stabilization, and facilitation of homelessness prevention and intervention programs, including Rapid Rehousing. Case Managers also assist participants with employment readiness, life skills training, financial education and budgeting, benefits enrollment, resource navigation, and individualized goal setting.

 

This is a community-based position that requires flexibility as to where services are provided. Case Managers are expected to meet participants where they are, which may include CRC offices, participants’ homes, community locations, housing units, partner agencies, and other locations. Regular local travel and reliable transportation are required.

 

Essential Functions and Responsibilities:

  • Provide information, resource navigation, basic crisis intervention, and eligibility screening to participants, callers, and visitors requesting assistance through CRC’s Social Services, Food and Nutrition, Domestic Violence, and Counseling Programs. Maintain a current understanding of the basic services and eligibility requirements of each program.
  • Manage an assigned caseload and provide individualized, housing-focused case management services. Complete intake and enrollment processes, collect and verify eligibility documentation, conduct psychosocial and needs assessments, determine program eligibility, develop and regularly update individualized case plans, monitor progress toward goals, and evaluate and submit participant requests for financial assistance.
  • Develop consistent and meaningful engagement with participants through scheduled case management sessions and ongoing follow-up. Use a flexible, participant-centered approach to engagement, including in-office, telephone, virtual, community-based, and in-home meetings based on participant needs and program requirements.
  • Conduct regular home visits, including visits following housing placement, to support housing stabilization, assess emerging needs or barriers, reinforce tenancy skills, and connect participants with appropriate resources and services.
  • Travel regularly throughout the County of San Diego to meet participants where they are, conduct home visits, view or inspect potential housing as appropriate, meet with landlords and community partners, and attend off-site meetings, and appointments.
  • Participate in activities directly related to contract outcomes, including administration of CRC housing and supportive services programs, benefits application assistance, housing placement and stabilization activities, and achievement of assigned program performance and compliance requirements.
  • Advocate on behalf of participants and coordinate services with landlords, property managers, healthcare and behavioral health providers, public benefit programs, and other community agencies. Research available resources, develop and maintain community partnerships, and make appropriate referrals to address participant needs and reduce barriers to housing stability.
  • Maintain accurate, thorough, and timely physical and electronic participant records in accordance with agency, grant, and contract requirements. Document participant contacts, services provided, progress toward goals, referrals, financial assistance, and other relevant case activity using required agency databases and documentation standards.
  • Exercise sound judgment, critical thinking, professional boundaries, and problem-solving skills when responding to participant needs, crises, housing barriers, and changing circumstances. Consult with supervisors when situations require additional guidance or intervention.
  • Attend and actively participate in individual and group supervision, case consultation, team meetings, trainings, and other meetings as required by the Social Services Program Manager and/or Director.
  • Other duties as assigned by the Social Services Program Manager and/or Director.

 

Qualifications & Requirements:  

  • Associate degree in Social Work, Psychology, Sociology, Human Services, or a related field required, Bachelor’s degree preferred. An equivalent combination of relevant education and experience may be considered. Candidates without an associate degree should have at least three additional years of relevant experience.
  • 1+ years of social services experience, including case management, crisis intervention, counseling, housing services, resource navigation or other direct client services.
  • Knowledge of homelessness prevention and intervention strategies, Rapid Rehousing or other housing programs, resources available to individuals and families with low incomes, Domestic Violence services, and Trauma-Informed Care practices is highly desirable.
  • Ability to work independently in both office and community-based settings while exercising sound judgment, maintaining appropriate professional boundaries, and seeking supervisory support when appropriate.
  • Demonstrated ability to remain professional, compassionate, solution-focused, and responsive when navigating challenging situations, crises, or competing priorities.
  • Strong organizational and time-management skills, including the ability to manage an active caseload, prioritize responsibilities, follow through on commitments, and meet documentation and program deadlines.
  • Ability to gather, assess, and synthesize information; identify needs and barriers; exercise critical thinking and problem-solving skills; and develop appropriate next steps in collaboration with participants.
  • Excellent written and verbal communication skills with strong attention to detail and the ability to maintain clear, accurate, timely, and professional case documentation.
  • Demonstrated ability to establish and maintain professional relationships with participants, landlords and property managers, community partners, service providers, and colleagues.
  • Experience with human services information systems or case management databases, such as Efforts to Outcomes (ETO) and Clarity/HMIS, preferred.
  • Proficiency with Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook, and ability to learn and effectively use agency databases and other technology required for the position.
  • Bilingual English/Spanish skills preferred.
  • Must successfully complete a 40-hour Domestic Violence Advocate training and other required agency or program-specific trainings within 60 days of hire, if not already completed.

Physical requirements :

  • Primarily sedentary with intermittent standing, walking, bending, and stair climbing.
  • Occasional light lifting and carrying of objects weighing up to 25 lbs.
  • Ability to work in a standard office environment at a computer, including repetitive use of a keyboard and mouse.

This position is full-time, non-exempt, and is paid according to federal and state laws. Candidates selected for hire are required to undergo a TB screening and a LiveScan background screening.
