Discharge Planner Opportunity

ARMStaffing is looking for a Discharge Planner in Morristown, NJ.

Location: Morristown, NJ

Assignment Duration: This is a 13-week contract position.

Schedule/Shift: 7.5 hrs/day, 37.5 hrs/week, 8:00 AM – 4:00 PM, rotating weekends (every 4th or 5th weekend), holidays (one summer and one winter holiday).

Job Description: The Discharge Planner will be responsible for managing the discharge process for patients, including coordinating placement in rehabilitation facilities. This role will involve working with subacute patients and ensuring a smooth transition to appropriate post-acute care.

Responsibilities:

• Develop and implement discharge plans for patients.
• Coordinate with healthcare teams to facilitate patient placements.
• Assess and evaluate patients for subacute rehabilitation needs.
• Assist patients and families with understanding discharge instructions and follow-up care.

Required Experience:

• Minimum of 2 years of experience with acute inpatient hospital discharge planning.
• Experience with subacute patients required.
• Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities.

Certifications/Licensure: Current licensure as a nurse or relevant healthcare professional is required.
