Afterhours Hospice RN Care Manager

- p Gentiva Health Services, Inc. is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform assessments, coordinate care, and maintain clinical oversight for patients and families during nights, weekends, and on-call shifts.
- p You will be part of the Interdisciplinary Group (IDG), update plans of care, educate families, document thoroughly, and ensure infection control and regulatory compliance while guiding field staff. This role requires an active RN license, a nursing degree, and at least one year of hospice or home health experience.
