Position: Occupational Therapist I

Job Description:

We are seeking a skilled Occupational Therapist to join our home health team. The successful candidate will be responsible for providing therapy services through home visits to patients. This role requires reliable transportation and the ability to meet patients in various locations.

Key Responsibilities:

• Conduct assessments and develop individualized therapy plans for patients in their homes.
• Manage a caseload of 5-10 patients per day, with an average of 5-6 patients each day.
• Participate in periodic office meetings and collaborate with other healthcare professionals.
• Document all patient interactions and therapy sessions within 24 hours.
• Travel up to 75 miles from the assigned clinic to visit patients as necessary.

Requirements:

• Minimum of 1 year of recent adult home health experience.
• Current CPR and BLS certifications.
• Must possess a valid driver's license and reliable transportation.
• Agency-required liability insurance is necessary.
• Proficiency in HomeCare HomeBase EMR and OASIS documentation is required.

Working Conditions:

This position operates on a 5-day schedule from 8:00 AM to 5:00 PM. On-call requirements may vary based on location and circumstances. Must be open to weekend coverage as needed.

Travel and Commuting:

The Occupational Therapist will be required to travel to patients' homes and the clinic for meetings, potentially leading to drives longer than 20 minutes between appointments. Mileage will be reimbursed at the IRS standard rate, although travel time to and from the first and last patient is not compensated.

Position Duration:

This is a contract assignment for 13 weeks with potential for renewal based on productivity and patient census.

Location:

Position will cover areas in Rockdale County and surrounding regions, specifically in Monroe, GA and Conyers, GA.
