RN - Case Manager (Backfill - Travel)

This is a 13-week contract position.

Shift: 8:00 AM - 4:30 PM, every other weekend required.

We are seeking an experienced RN Case Manager for a travel position. The successful candidate will be responsible for effectively managing patient care throughout the acute care setting, ensuring high-quality standards and optimal outcomes.

Responsibilities:

• Manage the care of patients through the case management process.
• Conduct assessments to identify patient needs and plan appropriate care interventions.
• Coordinate with healthcare providers to facilitate care transitions and resources for patients.
• Monitor patient progress and adjust care plans as necessary.
• Utilize Meditech, Midas, and Interqual for managing patient data and care documentation.

Required Experience / Certifications / Licensure:

• Acute (hospital) Case Management experience is required.
• Proficiency in Meditech, Midas, and Interqual is necessary.
• Valid RN license required.
