Contract | Full-Time | Rotating Shifts (Days/Evenings) | Onsite

Key Responsibilities

• Provide clinical pharmacy services in an inpatient hospital setting across rotating day and evening shifts
• Perform pharmacokinetic dosing, with a focus on vancomycin
• Manage anticoagulation therapy, including warfarin management
• Maintain coverage for weekend shifts as needed, with variable hours depending on staffing

Required Qualifications

• Active Ohio Pharmacist license in good standing
• Minimum of one year of inpatient hospital pharmacy experience
• Demonstrated experience with pharmacokinetic dosing, specifically vancomycin
• Experience with warfarin management
• Availability to work rotating shifts, including evenings (typical shift 12:30 PM – 9:00 PM) and weekends

Preferred Experience / Skills

• Experience with Epic Willow Inpatient pharmacy system
