Home-Visit NP/PA Clinician — Flexible Schedule

Optum is seeking an HouseCalls Advanced Practice Clinician (NP/PA) to conduct in-home health assessments for Medicare Advantage members. Flexible scheduling, no weekends or holidays, no on-call, travel required to patient homes across the region. You will perform comprehensive assessments, document diagnoses, educate patients, and coordinate with PCPs to close care gaps. Active state licensure and NP/PA certification required; driver's license and reliable transportation essential. #J-18808-Ljbffr Optum
