TRIUNE Health Group is seeking a Nurse Case Manager to coordinate resources and create flexible, cost-effective options for catastrophically or chronically ill or injured individuals, enabling timely return to work when appropriate. The role involves case management across patients, physicians, employers, and referral sources, with emphasis on communication, compliance with HIPAA, and driving optimal outcomes.
