Key Responsibilities

• Performing in-depth patient evaluations, ordering necessary diagnostic tests, and developing tailored care strategies
• Diagnosing and addressing acute, chronic, or episodic health issues either independently or in collaboration with physicians (conditional on credentialing)
• Imparting education to patients and families regarding prevention, wellness, and effective disease management
• Prescribing appropriate medications and therapies in accordance with established guidelines
• Engaging in patient follow-ups and chronic care management
• Collaborating with interdisciplinary teams to facilitate comprehensive and holistic care
• Ensuring meticulous and accurate documentation in electronic medical records (EMR)

Qualifications Needed

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner educational program

Certifications Required

• Current Nurse Practitioner licensure in New York State
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to obtain within 90 days)
