Minimum Qualifications Associate degree in Physical Therapy from an accredited educational program. Active Illinois PTA license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in a skilled nursing facility preferred. Experience with restorative nursing programs, ADL retraining, and assistive devices preferred. Minimum 1 year of recent PTA clinical experience preferred.

Assignment Details Facility Type: Skilled Nursing Facility. Shift: Days. Shift Hours: 08:30–16:30. Work Pattern: Five 8-hour shifts per week. Contract Length: 13 weeks (VMS Contract Weeks). Guaranteed Hours: 35 hours per week (VMS Guaranteed Hours).
