Job Description

The Occupational Therapist-Home Health in Maricopa County East Valley as well as Pinal County plays a key role in delivering personalized therapy services to primarily adult patients within their homes. As part of an interdisciplinary team, this position involves frequent travel to multiple residences, focusing on patient assessment, care planning, and implementing effective therapeutic strategies to enhance patients’ daily living skills. The therapist also educates patients and caregivers, monitors progress, and ensures safety through appropriate recommendations and documentation.

 

Responsibilities

• Conduct patient assessments to identify therapy needs
• Develop individualized care plans tailored to patient goals
• Implement therapeutic techniques during home visits
• Educate patients and caregivers about therapy and safety
• Monitor and document patient progress regularly
• Collaborate with interdisciplinary team members for coordinated care
• Evaluate home environments for safety risks and modifications
• Recommend appropriate therapy equipment and assistive devices

 

Preferred Qualifications

• Minimum 1 year experience in occupational therapy
• Licensed Occupational Therapist (OT)
• Strong skills in patient assessment and care planning
• Proficiency in therapeutic techniques and home health care
• Excellent communication, time management, and problem-solving abilities
• Demonstrated empathy and patient-centered approach
• Thorough documentation skills
