Bilingual Community Health Navigator

Tapestry 360 Health is looking for a Community Health Coordinator to support non-English-speaking patients and their families in Chicago, IL. The coordinator will assist with language interpretation, care coordination, and outreach activities to bridge communication gaps. This is a part-time, temporary position, requiring 20 hours of work per week. The ideal candidate has at least a year of experience in community programs or healthcare, and fluency in Dari/Pashto or Rohingya/Burmese is highly desired. #J-18808-Ljbffr Tapestry-360-Health
