Facilitate communication among patients, families, and healthcare providers; coordinate transitions between care settings and plan for post-hospital care. Evaluate medical necessity for treatments and hospital stays; collaborate with insurers to obtain authorizations and identify discharge barriers. Advocate for patients’ needs and preferences; educate patients and families about diagnoses, treatments, and self-care; connect with community resources. Maintain thorough documentation of assessments, interventions, and outcomes; participate in interdisciplinary care conferences and coordinate plan implementation across disciplines.
