Mobile Nurse Practitioner - Adult/Geriatric

Practice beyond the walls of a traditional clinic as a full-time Mobile Nurse Practitioner serving patients throughout San Gabriel Valley and East Los Angeles, California. In this role, you will bring high-quality, relationship-based care directly to patients through home visits, community-based care, and virtual appointments while working within a patient-centered, value-based care organization focused on improving outcomes and supporting long-term health.

Our care model is focused on quality outcomes, preventive care, proactive interventions, and coordinated patient support rather than high visit volume. You will enjoy a high level of clinical autonomy while working alongside an interdisciplinary team dedicated to helping patients achieve better health outcomes.

Position Highlights

Full-Time Schedule: Monday–Friday | 8:00 AM–5:00 PM

Competitive Base Salary: $130,159-$195,238

Qualifications

Active and unrestricted California Nurse Practitioner license

Active DEA license

Current BLS certification

Strong clinical assessment and diagnostic skills

Ability to practice independently while collaborating effectively with an interdisciplinary care team

Commitment to patient-centered, preventive, and value-based care

Excellent communication and relationship-building skills

Comfortable providing care in home, community, and virtual settings
