We are seeking a Nurse Practitioner or Physician Assistant to join our Family Medicine team in Chandler, TX . This role offers the opportunity to provide comprehensive, patient-centered primary care in a collaborative outpatient setting. The ideal candidate will be committed to building long-term patient relationships and delivering high-quality care to patients across all ages.

\n Key Responsibilities\n

\n
• Provide full-spectrum primary care including preventive services, acute care, and chronic disease management
\n
• Perform patient evaluations, wellness exams, and routine screenings
\n
• Order and interpret labs, imaging, and diagnostic studies
\n
• Develop and manage individualized treatment plans in collaboration with supervising physicians
\n
• Educate patients on disease prevention, medications, and lifestyle modifications
\n
• Maintain accurate and timely documentation within the EMR
\n
• Collaborate with physicians and clinical staff to ensure coordinated patient care
\n

\n Qualifications\n

\n
• Current Texas license as a Nurse Practitioner or Physician Assistant
\n
• National certification (AANP, ANCC, or NCCPA)
\n
• Previous family medicine or primary care experience preferred
\n
• Strong clinical judgment and communication skills
\n
• Ability to work independently and as part of a collaborative team
\n
