A Skilled Nursing Facility (SNF) Admissions Nurse (LPN) oversees the seamless transition of new residents into a facility. They conduct clinical pre-admission assessments, verify insurance, coordinate with hospital discharge planners, and ensure all care plans and medical records are accurately completed to provide a welcoming, compliant start. 

Key Responsibilities

• Clinical Assessments
• Coordination & Outreach: Act as the primary liaison between hospitals, physicians, the resident, and their family to ensure a smooth, stress-free transition.
• Documentation & Compliance: Ensure all admission paperwork, insurance verifications, and medical records are accurately completed and comply with state and federal regulations.
• Input od Orders/Medication Orders
• Patient Education: Review services, policies, and care processes with the new resident and their family members.

Typical Requirements

• Education: Graduate of an accredited nursing program.
• Licensing: Active, unencumbered Licensed Practical Nurse (LPN)  
• Experience: 1 to 2 years of nursing experience, preferably in long-term care, geriatrics, or case management.

Must Pass a Level II Background Check through the Clearinghouse

QUALIFICATIONS:  

• Must be licensed or registered in the state employed.

• Completion of post-secondary education and be able to read, write, and follow oral and written directions.  

• Must be capable of maintaining regular attendance as required.

• Must be able to work weekend and overtime as required.

• Must meet all local health regulations and pass post-employment exam, if required.

• Related administrative experience at a level necessary to accomplish this position.

• Must have and pass a Level II Background Check through the State of Florida Clearing House
