Hospice RN Case Manager: Balanced 15-Patient Caseload

Vitality Hospice

Vitality Hospice is seeking a dedicated RN Case Manager in Bucks and Montgomery County, PA. The role is Monday–Friday, 8:00 AM–5:00 PM, with rotating on-call and a ~15-patient caseload. You will serve as the clinical lead for your assigned patients, conducting comprehensive nursing assessments, developing and managing individualized plans of care, and collaborating with physicians, LPNs, social workers, chaplains and aides. #J-18808-Ljbffr
