The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. The Bureau of Harlem Neighborhood Health (Harlem BNH) supports the agency's mission to protect and promote the health of all New Yorkers. Harlem BNH catchment area includes the entire community of Harlem, including East, Central and West Harlem as well as Washington Heights and Inwood. Harlem BNH focuses on the structural and root causes of health disparity outcomes and develop programs to address these outcomes with input from partners and residents. Addressing the social determinants of health (SDOH), is a key focus point and is enshrined in programs such as the Harlem Health Advocacy Partnership and the Asthma Counselor Program. Other Bureau offerings include a variety of programs and activities focused on the health and wellness of Harlem residents. The Harlem Bureau also houses the East and Central Harlem Neighborhood Health Action Center, which is a key part of the NYC's effort to promote health equity and reduce health disparities in the neighborhood. The Bureau of Harlem Neighborhood Health seeks to hire a Program & Data Manager, Creating Healthy Schools & Communities (CHSC) to be the senior programmatic and operational lead for the Harlem Bureau of Neighborhood Health's CHSC programs, a NYS Department of Health funded initiative. The program focuses on integrated school- and community-based strategies that advance chronic disease prevention, physical activity, nutrition access, and health equity. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO Systems Building & Data Management Oversee data collection, monitoring, and reporting for programmatic, community engagement, and evaluation activities. Identify and disseminate best practices. Utilize Public Health Partners Connect and other Harlem Bureau data systems to track participation, partnerships, and outcomes. Integrate CHSC data with broader Harlem Bureau reporting systems. Prepare reports, dashboards and presentations for internal teams, funders and external partners. Present neighborhood health data to partners to inform shared strategies and collective action. Community & School Partnership Development Develop and maintain strong relationships with schools, community-based organizations, city and state agencies, and local community leaders. Build and support coalitions to leverage resources, strengthen coordination, and increase program sustainability. Provide training and technical assistance to partners implementing CHSC-aligned activities. Program Implementation & Grant Management Serve as programmatic and operational lead for day-to-day planning, implementation and monitoring of CHSC within Harlem Bureau. Manage all grant-related activities including budgeting, reporting, contracting, staffing and compliance. Translate grant deliverables into actionable workplans aligned with Bureau and community-wide priorities. Support program sustainability and continuous improvement efforts. Coordinate vendors, facilitators and partners supporting physical activity, nutrition and wellness initiatives. Support integrated school-community programming that reflects neighborhood needs and lived experience. Manage staff. Complete special projects and initiatives. Represent Bureau of Harlem Neighborhood Health at meetings as needed. Some evenings and weekends required. For Assignment Level I (only physical, biological and environmental sciences and public health) A master's degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health. To be appointed to Assignment Level II and above, candidates must have: 1. A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or 2. A master's degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or 3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least a master's degree in an appropriate field of specialization and at least two years of experience described in "2" above. Two years as a City Research Scientist Level I can be substituted for the experience required in "1" and "2" above.
