• Conduct thorough clinical assessments of patients to identify their psychosocial needs and formulate individualized treatment plans.

• Implement evidence-based therapeutic interventions, focusing on emotional support and grief counseling for patients and their families.

• Maintain accurate and timely clinical documentation, ensuring compliance with regulatory requirements and best practices in hospice care.

• Collaborate with a multidisciplinary team, including physicians, nurses, and spiritual care providers, to deliver holistic care that addresses all aspects of our patients' well-being.

• Educate and support patients and family members about the emotional journey of end-of-life care, equipping them with strategies to cope with stress and grief.

• Stay current with ongoing research, treatment modalities, and best practices in geriatric and end-of-life mental health care.

This role focuses on serving a diverse patient population, including individuals facing terminal illnesses and their families. • A Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Florida.
