RN Home Health, local or travel, Culver City, CA - 13 weeks. ASAP start! Mon - Fri, 8am - 5pm, 40 hours per week. 13 weeks. Pay rate plus IRS mileage. Start around end of May to replace current traveler. 13 weeks M-F, days. Weekend rotation: 1 weekend/month with day off. Coverage area: Santa Monica/Marina Del Rey, Beverly Hills, Brentwood. May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the facility address. RN productivity expectation for 40 hrs = 28 - 32 points/wk. SOC 2.5, ROC 2.0, Discharge (OASIS) 1.5, Recertification - 1.5, Routine visits - 1. Must have recent HH exp 2023-24 HomeCare HomeBase experience required. Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train. Any overtime must be pre-approved and requires written approval from Administrator. 2 weeks cancellation.
