Social Worker, MSW or LCSW - Home Health Visits
Type: PRN/Per Diem
Location: West Volusia County
Pay: $65 per visit

Provide high-quality, compassionate care to home health patients
Work under the direction of the Director of Clinical Services and Clinical Team Manager
Deliver assigned functions of care in a professional and patient-focused manner
Utilize knowledge of community, state, and federal resources to support patients
Make a meaningful impact in patients' lives every day

What You Bring:
Master's degree in Social Work (MSW) or Licensed as a Clinical Social Worker (LCSW) from an accredited school required for consideration
1-2 years of experience required, with strong community knowledge
1+ year of home care experience preferred
Ability to exercise initiative, sound judgement, and independent decision-making
Strong collaboration and communication skills
