Compensation & Benefits
  • Salary Range: $110 - $125K annually
  • Bonus/Incentives: Management Incentive Plan (10% target; up to 15% max)

Job Summary

A leading health system in Houston is seeking a Manager, Trauma Case Management to support the Director of Case Management in overseeing operations within a high-acuity trauma environment.

This role leads care facilitation, utilization management, discharge planning, and quality improvement initiatives to drive optimal clinical and financial outcomes. The Manager partners with multidisciplinary teams to ensure effective care coordination and patient flow throughout the continuum of care.

Key Responsibilities
  • Oversee daily operations of the Trauma Case Management program
  • Lead utilization management, discharge planning, and care coordination processes
  • Optimize patient flow and resource utilization in a trauma-focused inpatient setting
  • Collaborate with physicians, nursing, trauma teams, and external partners
  • Monitor and analyze financial, quality, and utilization metrics
  • Support hiring, training, and performance management of staff
  • Implement performance improvement initiatives using data-driven strategies
  • Serve as a liaison across interdisciplinary teams to improve communication
  • Ensure compliance with regulatory standards and organizational policies
  • Build and lead a high-performing, patient-centered team
Minimum Qualifications

Education:
  • Bachelor's Degree in Nursing (BSN) or Social Work (BSW) required
  • Master's Degree preferred
Licensure/Certifications:
  • Active Texas RN license OR Licensed Master Social Worker (LMSW) required
  • LCSW preferred (if applicable)
  • Case Manager Certification required
Experience:
  • Minimum 5 years of case management, utilization management, or discharge planning experience
  • Minimum 3 years of hospital-based clinical experience (trauma experience highly preferred)
  • Minimum 3 years of leadership or supervisory experience
Preferred Skills
  • Strong understanding of trauma workflows and acute care operations
  • Knowledge of patient throughput, managed care, and post-acute transitions
  • Experience with performance improvement and outcome-based initiatives
  • Proven collaboration skills with physicians and clinical teams
  • Strong leadership, communication, and organizational skills
