Registered Nurse (RN) Per Diem

Description The job offers the opportunity to provide exceptional hospice care through skilled nursing services that support patients and families during their end-of-life journey. The Registered Nurse (RN) will work independently and collaboratively with an interdisciplinary clinical team to deliver high‑quality, personalized care within the guidelines of state and federal regulations. Schedule Schedule : Must be available for at least 3 days per month, with flexibility to add hours for on‑call shifts during evenings, weekends, and holidays. Locations Locations : Travel to Sacramento, Yolo, Solano, Colusa, and Sutter counties as needed for coverage and support. The YoloCares Clinical Team The RN will join a highly skilled, compassionate interdisciplinary team of nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who collaborate to support patients and families throughout the hospice journey. Position Summary Under the direction of the Clinical Manager, the RN will provide skilled nursing care that adheres to each patient’s evolving Plan of Care and to YoloCares policies. Documentation and communication of nursing services will be maintained in a timely manner to support continuity of care. Key Responsibilities Clinical Care Complete nursing assessments for assigned patients. Collaborate with medicine, social work, and spiritual care counseling to meet care goals. Coordinate necessary medications, durable medical equipment, and supplies, ensuring cost‑effective delivery. Apply Standard Precautions in all direct patient care. Provide ongoing professional nursing care, regularly reassessing needs and reporting findings. Arrange scheduled visits or additional visits as patient status or family needs change. Documentation Ensure all patient documentation is complete, accurate, and reflects assessments, interventions, and goals. Update or complete a patient Plan of Care to reflect problems, plans, actions, and goals. Obtain, document, and submit medical orders promptly. Maintain visit notes that clearly reflect nursing interventions and goals. Care Coordination and Changes in Condition Communicate with the attending physician or designee regarding changes in patient condition and emergent needs; obtain necessary orders. Collaborate with supervisor to select appropriate level of care (Continuous, Respite, or General In‑Patient) and for decertification or discharge. Patient and Family Support Teach needed nursing care and support skills to patients and family caregivers. Provide instructions on medication, activity, diet, symptom management, safety, expected disease progression, and the dying process. Offer after‑care support at the time of death. Ensure all staff are promptly informed of patient death. Collaboration and Communication Attend required clinical meetings and help develop standards of care. Maintain professional relationships with other health‑care providers and programs. Report issues with referral providers that may need intervention. Case Management Assume responsibility for assigned patient caseloads and provide case‑management within YoloCares standards. On‑Call and Weekend Responsibilities Triage after‑hour calls, determine necessity of visits, and coordinate as required. Inform staff of calls and visits at the start of next shift and document visits. Participate in rotating weekend and on‑call shifts. Maintain a flexible schedule to ensure timely, responsive care. Requirements Demonstrated passion for hospice care and commitment to compassionate support. Current California RN license in good standing and CPR certification. Effective verbal and written communication in English; Spanish is a plus. Three (3) years of recent clinical nursing experience; home or hospice care experience required. Strong clinical nursing skills and understanding of hospice philosophy and goals. Self‑motivated, autonomous and collaborative, with initiative, prioritization, and organization skills. Ability to make appropriate clinical decisions and collaborate with community physicians and other disciplines. Capacity to handle frequent interruptions and complete duties efficiently. Supervise Hospice Aides (prior management experience not required). Pass background screening requirements. Physical Requirements Lift up to 35 pounds and assist with transfers and ambulation; extensive walking and standing may be required. Meet vision and hearing standards necessary for nursing duties. Maintain and drive a personal vehicle; hold a valid California driver’s license and required insurance when on business. Perform home visits and CPR as needed. Schedule Requirements Must be flexible to add work hours for on‑call shifts during evenings, weekends, and holidays. Work a minimum of three (3) days per month for YoloCares. #J-18808-Ljbffr
