Responsibilities Assess physical, emotional, socio‑economic, and environmental needs of patients and families. Provide services ordered by the physician as indicated in the plan of care. Communicate with all physicians involved and other health‑care practitioners as appropriate. Develop, implement, and evaluate the plan of care in partnership with the patient, representatives, caregivers, and physicians. Coordinate services with the patient, family, and others involved in the care plan. Provide patient, caregiver, and family counseling and education. Regularly evaluate patient progress, take prompt action when changes in condition are noted or reported, and terminate care when goals are attained or agency policy requires termination. Participate in the agency quality assurance and performance improvement program, and attend agency‑sponsored in‑service training. Identify patient and family needs for other home health services and refer appropriately. Supervise services rendered by homemaker/home health aide in accordance with state, federal, Joint Commission standards, and submit periodic evaluations to the Home Health Aide Supervisor. Fulfill all compliance responsibilities, including preparing clinical notes and attending in‑service training. Qualifications RN diploma with over 3 years of job‑related experience considered in lieu of an Associate’s Degree. Current RN license in the state of Connecticut. Strong research skills, clear and concise interpersonal communication, and knowledge of systems integration. Minimum one year of experience; desired experience in a qualified home health agency. Knowledge of operating in a home health environment (manual skills, motor coordination, finger dexterity).
