Care Coordination RN Opportunity

This respected hospital is seeking a Care Coordination RN to support safe, well-coordinated transitions of care across the continuum. Ideal for a collaborative RN who values clear communication, strong teamwork, and measurable outcomes. In an accredited acute care setting with approximately 350 beds, the Care Coordination RN will join a not-for-profit organization recognized for quality, safety, and patient experience. Strong clinical programs and interdisciplinary partnerships help the Care Coordination RN contribute to reliable, patient-centered care.

The Care Coordination RN reports to the Director of Care Management and Utilization Review and partners closely with the Vice President of Population Health and hospitalist leadership. The role collaborates with bedside RNs, social workers, case managers, physicians, pharmacists, rehabilitation therapists, and revenue cycle/UM specialists to move care forward. As needed, the position may precept colleagues and help align daily operations with departmental priorities.

Within the Care Management Department, the Care Coordination RN supports all inpatient units, the emergency department, and perioperative areas in a steady, team-focused environment and works alongside experienced social workers and physician leaders dedicated to discharge readiness, readmission reduction, and denial prevention. Using real-time data and established pathways, the Care Coordination RN helps coordinate care that is timely, clear, and safe for patients and families.

The Care Coordination RN conducts comprehensive assessments, identifies risks, and coordinates plans spanning patient/caregiver education, aftercare, and payer communication. Additionally, the role manages utilization review (InterQual/Milliman), admission and continued-stay reviews, denial mitigation, and goals for length of stay, readmissions, and satisfaction, while documenting clearly, communicating proactively, contributing to performance improvement, and ensuring safe transitions across inpatient, ED, surgical outpatient, and geriatric settings.
