What You’ll Do Perform comprehensive in-home physical therapy evaluations Develop and implement personalized care plans that drive real results Deliver evidence-based interventions including: Gait training Strengthening Balance & fall prevention Pain management Functional mobility Empower patients and caregivers through education and support Monitor progress, update goals, and adjust interventions for optimal outcomes Collaborate with physicians, nurses, and interdisciplinary teams Maintain accurate, timely, and compliant documentation What We’re Looking For Licensed Physical Therapist (PT) – Texas Current CPR Certification (or willingness to obtain) Home Health experience preferred Reliable transportation + valid driver’s license Strong clinical decision-making skills Excellent communication and documentation abilities Self-motivated with the ability to work independently Who You Are Compassionate, patient-centered, and purpose-driven Clinically sharp with a commitment to excellence Organized and efficient with strong time management Goal-oriented and results-driven A team player who values collaboration and integrity Adaptable in a fast-paced, evolving healthcare environment Ready to Make an Impact?
