Functions as member of interdisciplinary healthcare team./ph3Experience/h3ulliTwo years/3,000 hours of experience with Bachelor of Science in nursing or three years/4,500 hours of experience with associates degree in nursing/li/ulh3Preferred Qualifications/h3ulliBachelor of Science in nursing/li/ulh3Education/h3ulliGraduation from an accredited school of nursing/li/ulh3Certification Summary/h3ulliLicensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact/liliBasic Life Support (BLS) certification from the American Heart Association within 30 days of employment/liliPediatric Advanced Life Support (PALS) or Pediatric Emergency Assessment Recognition and Stabilization (PEARS) from the American Heart Association within 1 year of employment as required by department/liliProfessional Nursing Certification as required by department/liliContinuous employment as a Clinical Nurse 3 at Childrens Healthcare of Atlanta since March 2017 in lieu of Professional Nursing Certification/li/ulh3Knowledge, Skills, and Abilities/h3ulliNo minimally required knowledge, skills, or abilities/li/ulh3Job Responsibilities/h3pPractice (Caring for the Patient)/pulliCultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care./liliCommunicates with all members of the multidisciplinary team to facilitate the care coordination process for specialty./liliCommunicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs./liliFacilitates and provides ongoing communication with patient/family, interdisciplinary staff and third parties (home health, third party payers, pharmacies, and other resources) to identify and resolve potential barriers./liliFacilitates clinical practice guidelines at the patient level./liliPerforms outpatient and clinic care coordination and monitors patients care between clinic and home./liliFacilitates care via telehealth to assist with patient needs and concerns./liliRefers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel./li/ulpEngagement (Engaging and Leading Professionally)/pulliProvides oversight for nursing care given by unlicensed personnel while retaining accountability for patient care quality./liliEngages in ethical practices utilizing principles from The Code of Ethics for Nurses./liliMaintains therapeutic and professional relationships with appropriate role boundaries./liliContributes to a supportive and healthy work environment./liliConsults, contributes to, and coordinates care with interdisciplinary healthcare team members.
