Continuous Care Nurses

Looking for a Skilled Registry Nurse/ LVN Nurse
Have an active license
Background check
TB - Physical- Drug test
Be able to work bedside, providing hands-on hourly care in a patient's home to manage acute symptoms.
Medication Administration: Administering and adjusting pain, symptom management, as well as intravenous (Iv)
medicaments as needed.
Monitoring Vitals: Take and record vital signs, including blood pressure, heart rate, and pulse oximetry hourly and continuously.
Patient Assessment: Ongoing Assessment - respond to significant changes in conditions to physicians or RN's
Documentation: Updated patient files to maintain compliance.

The primary goal of a continuous care nurse is to maintain the patient's comfort and safety at home.
