• Assignment Length: 13-week travel assignment
• Schedule: Monday-Friday, 8:00 AM-4:30 PM
• Shift Duration: 8-hour days
• Hours: 40 hours per week, including a 30-minute lunch
• Weekend Requirement: No weekends

Licensed Master Social Worker Job Requirements

• Active Licensed Master Social Worker (LMSW) license required
• Healthcare social work or discharge planning experience preferred
• Strong communication, documentation, and care coordination skills
• Ability to work effectively with interdisciplinary healthcare teams
• Knowledge of transitions of care and patient support services

Social Work Responsibilities

• Coordinate and support discharge planning for patients in a healthcare setting
• Collaborate with clinical staff, case management, and interdisciplinary teams to ensure safe transitions of care
• Assess patient needs related to discharge planning, community resources, and support services
• Document interventions, care plans, and patient progress accurately and thoroughly
• Provide resource coordination and patient advocacy throughout the care continuum
• Support evolving departmental needs as responsibilities change over time; psychosocial duties are not currently part of the role but may be added in the future
