About the Position

Job Title: Care Manager Profession: Registered Nurse Specialty: Emergency Department Duration: 14 weeks Shift: 9:00 AM - 5:00 PM Hours per Shift: 35 Experience: - Minimum of five (5) years of clinical experience as a Registered Nurse. - Experience with care coordination, utilization review, and discharge planning. License: - Licensed as a Registered Nurse in the State of New York. - Primary source verification of license and registration from the NYS Department of Education. Certifications: - Basic Life Support (BLS) certification. - NYSDOH H/C PRI training certificate required by submission or obtained post-offer. - PRI and Screen certification within three months of employment is required. Must-Have: - Bachelor's Degree from an accredited nursing program is preferred. Description: The Care Manager will oversee care coordination and discharge planning efforts. This role requires strong computer literacy and clinical nursing skills. The candidate should possess a comprehensive understanding of utilization review processes. Leadership experience is preferred. Each candidate must demonstrate a commitment to providing high-quality patient care. The successful applicant will work collaboratively with healthcare teams to ensure optimal patient outcomes.
