Visiting Nurse Association of Central Jersey Community Health Center (VNACJ CHC) is a federally qualified health center in Monmouth County, dedicated to offering accessible, high-quality healthcare to New Jersey residents. Position Summary:

The Family Nurse Practitioner provides comprehensive primary care services to patients within our Pediatric and Primary Care departments in Freehold and Keyport, NJ , with a significant focus on pediatric care. This full-time role is responsible for evaluating, diagnosing, and treating acute and chronic conditions, delivering preventive care, and promoting patient wellness across the lifespan. The ideal candidate will have outpatient pediatric experience, strong communication skills, and a passion for supporting children and families. Bilingual English/Spanish skills are strongly preferred to help meet the needs of our diverse patient population.

Principal Responsibilities:

• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers

• Provides comprehensive primary care services to pediatric and adult patients, including evaluation, diagnosis, treatment, preventive care, and management of acute and chronic conditions

• Provides client teaching to individuals and families when appropriate

• Monitors quality and appropriateness of services provided

• Identifies new services or programs relevant to agency’s mission and philosophy

• Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies

• Supervises staff in conjunction with the organization’s managerial and leadership practices

Job Competencies & Minimum Qualifications:

• Licensure as an Advanced Practice Nurse in NJ

• Master’s degree in Nursing from an NLN accredited program

• Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA

• 2+ years of primary or ambulatory care nursing experience required

• Business or management degree or experience preferred with demonstrated leadership skills

• Car and valid driver’s license

Working Conditions/Physical Demand: Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $120,000 to USD $146,000.
