Nurse Case Manager I

Position Title Nurse Case Manager I Location Texas (candidate must reside within the state of Texas) Anticipated End Date 2026-08-14 Role Summary This field‑based role focuses on providing care management services to members with complex and chronic needs. The Nurse Case Manager I will assess, develop, implement, coordinate, monitor and evaluate care plans to optimize member health care across the continuum. The role includes telephonic and on‑site interactions at hospitals, members’ homes, or clinics to reduce hospital/ER readmission. The position requires field presence 2‑3 times per week based on business needs, with occasional office attendance for meetings or training. How You Will Make an Impact Ensures member access to appropriate services based on health needs. Conducts assessments to identify individual needs and develop a specific care management plan. Implements the care plan by facilitating referrals within benefits structure or through state contractual arrangements. Coordinates internal and external resources to meet identified needs. Monitors and evaluates the effectiveness of the care management plan and modifies it as necessary. Interfaces with Medical Directors and Physician Advisors on the development of treatment plans. Assists in problem‑solving with providers, claims or service issues. Minimum Requirements BA/BS in a health‑related field and a minimum of 3years of clinical experience; or equivalent education and experience. Current, unrestricted RN license in the state of Texas. Multi‑state licensure if providing services in multiple states. Preferred Skills, Capabilities and Experiences Certification as a Case Manager. Experience with Microsoft Teams, Outlook, Word, and Excel. Experience with home health field visits. Experience with Medicaid. Positive, strong interpersonal and communication skills.
