RN In Case Management
As an RN in Case Management, you will be responsible for assessing and coordinating the care of patients, ensuring their discharge plans are effective and timely. You will work collaboratively with a multidisciplinary team to address psychosocial and medical needs, while managing a patient ratio of 25:1. This role offers a structured schedule with rotating weekends and no call requirements, allowing for a balanced work-life experience.
