Role Description The Hospice Clinical Liaison is a full-time, on-site role based in the Dothan, Alabama Area, responsible for connecting patients, families, and healthcare partners with appropriate hospice services. Day-to-day, the Clinical Liaison educates physicians, hospital staff, and community providers about hospice care, conducts clinical assessments, and facilitates timely referrals and admissions. This role coordinates closely with the interdisciplinary hospice team to ensure smooth transitions of care, clear communication, and accurate documentation. The Clinical Liaison explains hospice benefits and care plans to patients and families, addressing questions and concerns with sensitivity and clarity. The position also involves building and maintaining strong professional relationships, participating in outreach activities, and representing Gentiva Hospice in the local healthcare community. Qualifications Clinical expertise in hospice, palliative care, or related fields, with the ability to perform patient assessments and interpret medical information. Strong communication, interpersonal, and patient/family education skills to explain hospice services and care options clearly and compassionately. Relationship-building and community outreach abilities to collaborate effectively with physicians, hospitals, and other referral sources. Organizational and time-management skills to handle multiple referrals, documentation requirements, and care coordination tasks efficiently. Knowledge of hospice regulations, reimbursement, and eligibility criteria, or willingness to develop this expertise through training. Current clinical license (e.g., RN, LPN/LVN, social worker, or other relevant healthcare license) in the state of Alabama, as required by role scope. Experience in acute care, home health, hospice, or case management is highly beneficial. Valid driver’s license and reliable transportation for travel within the Greater Birmingham, Alabama Area.
