Choice Hospice Nurse Practitioner
The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predisposed to thrive in an unstructured environment and is okay with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members.
As a Hospice Nurse Practitioner, you will:
- Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
- Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
- Perform Face-to-Face visits at the patient's place of residence.
- Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
- Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
- Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
- Knowledgeable of hospice criteria.
Requirements
- Current STATE RN and Advanced Practice Nurse License
- Valid Driver's License
- Valid Auto Insurance
- AHA CPR Certification
- HomeCare HomeBase experience preferred
