Your team includes: Gather Guides community-based partners who address social determinants of health and connect patients to resources Medical Technician Partners (EMTs/Paramedics) deployed into patients' homes for post-discharge follow-ups, chronic condition monitoring, and home safety assessments, reporting directly back to you Medical Assistants, Patient Benefits Partners, and Practice Operations Partners handling the administrative and clinical support so you can focus on care Schedule flexibility built in Administrative days can be completed remotely.
