Nurse Practitioner

Oak Street Health seeks a compassionate Nurse Practitioner to provide comprehensive primary care to adults on Medicare in underserved communities. You will perform assessments, diagnose and manage acute and chronic conditions, develop preventative care plans, and coordinate services within a multidisciplinary team. Responsibilities include patient education, medication management, ordering and interpreting diagnostics, and maintaining accurate EMR documentation.
