What You’ll Be Doing Provide advanced wound care services, treatment planning, and ongoing management of complex wounds. Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long‑term care communities. Manage medications, diagnostics, treatment plans, and follow‑up care. Collaborate closely with physicians, nurses, therapists, caregivers, and facility leadership. Support preventative care initiatives and continuity of care efforts. Help improve healing outcomes while reducing avoidable complications and hospitalizations. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred). Experience in wound care, geriatrics, primary care, post‑acute care, or skilled nursing preferred. Strong clinical judgment and confidence managing medically complex patient populations.
