Physical Therapist

Job Title: Home Health Physical Therapist Location: Pulaski, TN Job Type: Part Time Position Overview The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on the assessment, the Physical Therapist determines a treatment plan, performs interventions aimed at improving the patient's well‑being, and evaluates progress. Responsibilities Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care. Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician. Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice. Instruct and inform patients, caregivers, and the health‑care team regarding the plan of care, functional limitations, and progress toward goals. Qualifications Current PT license in the state where services are rendered. Current CPR certification, driver’s license, and vehicle insurance with access to a dependable vehicle or public transportation.
