Assistant Director of Nursing

Hampton Court Nursing and Rehabilitation Center

As an ADON / RN you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties. As the ADON, you are the second in command of the nursing department. You will work closely with the Director of Nursing (DON) to improve the quality of patient care and services provided by your team. In the DON’s absence, you will oversee the entire team ensuring quality customer service and compliance with all federal, state and local rules and regulations, as well as the policies and procedures set by your employer. The Assistant Director of Nursing Services compliments the Director of Nursing Services in planning, developing and supervising the activities of licensed and non-licensed personnel who provide health care and nursing services to Nursing Home residents. This position assists with the development and implementation of nursing services, objectives, policies and procedures in accordance with Federal and State regulations. The incumbent shares, with the Director of Nursing Services, the responsibility for selecting Nursing Department staff, completing employee appraisals and resolving problems involving employee disciplinary action. Such supervision must be in accordance with your independent judgment, current federal, state and local standards, Florida RN Nurse Practice Act and regulations that govern our facility and as may be required by the Director of Nursing Services to ensure that the highest degree of quality of care and employee performance is maintained at all times. The following duties, responsibilities and functions have been determined by Hampton Court Nursing and Rehabilitation Center to be essential to the successful performance of this position. RESIDENT INTERACTION Talks to and interacts with residents respectfully; be knowledgeable of and sensitive to residents verbal and nonverbal communication. Provide guidance, counseling and assistance in order to engage residents in ways that are relevant, purposeful and in tune with resident choices, interests and concerns. Guide and assist residents in the activity of daily living needs such as bathing, dressing, feeding, transfers, etc. Support and facilitate resident relationships with family members, friends, guardians and other advocates. Support each resident’s personal and family religious belief system and facilitate residents’ attendance at religious services according to resident choice. Be attentive to and respect residents’ desires and cues for personal space and privacy. Interact professionally and appropriately with residents in regard to attitude, self-control, self-discipline, cooperation, and tone of voice, language and usage and in other areas as needed. Work with residents to discover and pursue personal goals in keeping with their risk management plans and care plans. Serve as a resident advocate by communicating effectively, incorporating resident values in plan of care, facilitating resident decision making and honoring and respecting resident rights and choice. Create and maintain an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment throughout the unit and shift. GENERAL FUNCTIONS, ROLES, RESPONSIBILITIES AND FUNCTIONS The Assistant Director of Nursing Services, or ADNS, oversees and manages the work and services provided by the nursing staff. The Assistant Director of Nursing Services shares responsibility for backing up RN/LPN/CNA staff on a 24-hour per day, 7 day per week basis. The Assistant Director of Nursing Services may confer with residents' physicians on an occasional basis to clarify medical orders and direct care. In the absence of the Director of Nursing Services, the Assistant Director of Nursing Services performs the duties of the Director. ADON is heavily involved in hiring and training staff. This includes a RN/LPN staff as well as the certified nursing assistants. You will likely put together a work schedule and directly supervise the staff by monitoring the distribution of medications, blood draws and other elements of patient care. You may also have the responsibility of maintaining a budget, as well as supplies and inventory. You will develop many reports for licensing and accreditors, and periodically plan and implement staff training seminars. Oversees the clinical operations (i.e. monitoring resident conditions, psychopharmacological drugs, restraint use, indwelling catheters, pressure ulcers, etc.) of the Nursing Department. Assists the Director of Nursing Services in the design, implementation and evaluation of nursing systems that ensure consistent delivery of care and maintains and promotes resident rights. Conducts nursing rounds on all units several times a day, including the beginning and end of shift. Provides reports/recommendations as needed/required for the Director of Nursing Services concerning the operations of Nursing Services. Assists the Director of Nursing Services in the development, implementation and evaluation of facility policies, procedures and guidelines on an annual and on-going basis (i.e. nursing, pharmacy, lab, physicians, medical records, etc.) In the absence of the Director of Nursing Services, conducts staff meetings. Assists the Director of Nursing Services in ensuring that residents are free from abuse, mistreatment and neglect. Assists the Director of Nursing Services in ensuring nursing staff awareness and compliance with Federal and State regulations relative to resident rights issues. The Assistant Director of Nursing Services provides leadership and supervision to the nursing department. Under the direction of the Director of Nursing Services, the Assistant Director of Nursing Services monitors nursing activity, communicates change, conducts performance evaluations and disciplines staff as needed. An important component of supervision is to consistently provide feedback to staff. This will help ensure that staff comprehends necessary changes to maintain the operations of the nursing department. Nursing departments require the effective handling of patient information, including medical, history and care-plan documentation. The Assistant Director of Nursing Services ensures the proper handling of personal health information in an effort to remain compliant with the Health Insurance Privacy and Portability Act. Although the Assistant Director of Nursing Services may seek the assistance of administrative duties such as filing, she is ultimately responsible for the accurate adherence of administrative compliance. An Assistant Director of Nursing Services is an expert with health care regulations specific to her area of expertise, such as long-term care or acute care. The Assistant Director of Nursing Services will ensure regulatory compliance by conducting mock surveys, interviewing patients and randomly assessing medical administration records. Regulatory compliance includes understanding health care rules and regulations at the local, state and federal levels. Despite the trend that administrative nurses, such as an Assistant Director of Nursing Services, spend the majority of their time completing administrative tasks, they are still responsible to provide patient care as needed. An Assistant Director of Nursing Services who is able to provide direct care to a patient displays a willingness to help others and a level of practical nursing skills. In the absence of other nurses or nursing assistants, the Assistant Director of Nursing Services is required to assist with patient care. Conduct the daily nursing functions in accordance with your independent judgment, current rules, regulations, and guidelines that govern the facility. Ensure that all nursing personnel assigned to you comply with the written policies and procedures including the safety program. Make rounds with health care provider. Utilize nursing personnel qualifications according to the resident’s care plan, wishes and health care provider orders. Complete daily clinical and environmental rounds to evaluate resident care being provided and report significant events. Meet and resolve issues with residents and/or family members. Report significant issues to your supervisor. May be responsible for clinical programs as directed, e.g. dementia, wound care, quality assurance, infection control, admissions. Update, review, and transcribe the care plan that identifies the goals, problems, approaches, and revisions based on nursing needs and new health care provider orders. Communicate with health care provider to review treatment plans, complete orders, progress notes, etc., in accordance with established policies. Review the resident’s medical record for specific treatments, medication orders, diets, labs etc., as for implementation and follow up. Requisition and arrange for diagnostic and therapeutic services, as ordered by the health care provider, and in accordance with our established procedures. Communicate with written (24 hour report) and oral reports/recommendations concerning any changes in resident condition and other activities of your shift to nursing management. Fill out and complete accident/incidents involving residents or staff and submit to Director of Nursing. Complete and file assessments and all other medical record documentation as required to provide care services. Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the supervisor. Complete clinically accurate documentation according to policy. Review the resident medical documentation and progress notes from the interdisciplinary team to determine if the care plan is being followed. Report any concerns to your supervisor. Inform nursing personnel of new admissions and the resident care needs. Ensure that rooms are ready for new admissions. Greet newly admitted residents upon admission. Notify the resident’s attending health care practitioner and family when there is a change in the resident’s condition. Meet with your assigned support personnel (Housekeeping, Dining Services, Nursing Assistants, etc.) to plan the shift’s services, programs, and activities and monitor tasks are completed as directed. Coordinate services with other members of the disciplinary team (Housekeeping, Dining Services, Nursing, etc.) to ensure that the resident’s total regimen of care is maintained. Assist in arranging transportation and appointments for residents and other third party services. Review medication administration record for completeness of information and accuracy.Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand during your shift. Adjust the staffing needs of the nursing service department necessary to meet the total nursing needs of the residents. Develop work assignments, schedule duty hours, and assist and supervise nursing staff to ensure that all personnel are performing their work assignments in accordance with acceptable nursing standards. Assist in staff development. Formally and informally monitor performance of personnel and ensure adjustments/corrections are made by using coaching, counseling, and discipline methods. Work with the Director of Nursing to resolve staff performance issues including those that may lead to termination. Review complaints and grievances, complete investigations, and work with the Director of Nursing for a plan of action and resolution. Participate in the planning, conducting and scheduling of timely in service and orientation training classes that meet state and federal requirements. Maintain nursing service work areas (i.e., nurses’ stations, medicine preparation rooms, etc.), resident rooms, and treatment areas in a clean and sanitary manner. Meet with and resolve personnel issues with the licensed practical nurses who you supervise. Recommend to the Director of Nursing the equipment and supply needs of the nursing department. Ensure that a stock level of medications, medical supplies, equipment, etc., is maintained on the premise at all times to adequately meet the needs of the residents. Integrate advanced knowledge and experience in delivering safe, effective quality care to the residents. Demonstrates competence in managing health / illness status of residents. Manage and negotiate within the healthcare delivery system on behalf of the residents. Monitor and ensure quality healthcare for the residents. Incorporate an understanding of trends in aging in planning and providing care. Demonstrate leadership and competence in implementing the role of an RN. Engage in counseling, communication, collaboration and teaching in a manner that reflects caring, advocacy, ethics, and professional standards. Understand patient acute and chronic illness and react accordingly. Interpret and evaluates diagnostic tests to identify and assess patient condition. Develops a plan and discusses it with the physician or designee. Consults and coordinates with health care team members to assess, plan, implement and evaluate patient care. Modifies patient treatment plan as indicated by patient responses and conditions. Monitors all aspects of patient care including diet and physical activity. Directs and supervises subordinate employees on a unit or throughout the facility. When dealing with ill or injured residents you are knowledgeable and proficient in your duties in managing IVs, ensuring residents and their family members understand the resident’s condition and treatments, supervise nursing staff, monitor fluid and food intake and output and move residents safely. Observe residents, chart findings, take necessary actions and report changes in patient condition in an efficient, effective and accurate manner. Provide resident care or treatments such as performing and recording temperature, pulse, respirations, blood pressures, dressing skin impairments, treating skin impairments, giving enemas, suctioning, inserting indwelling catheters, etc. Answer resident call lights, determine how to assist resident and ensures that resident is assisted that is satisfactory to the resident need and choice. Assemble and use equipment safely and accurately under the standards of nursing for catheters, trach tubes, oxygen, supplies, etc. Evaluate nursing intervention outcomes and confers with other health care team members as necessary. Observe, receive and obtain information from relevant sources to complete tasks through collection of clinical data and obtaining information from other staff members, family members, residents, vendors, physicians, etc. Provide information, analyze and evaluate results to choose the best solution and solve problems in making decisions as it relates to your duties, responsibilities, roles and functions. Provide information to supervisors, coworkers, physicians, etc. by telephone, in written form or in person in an accurate and timely manner. Identify information by categorizing, estimating, recognizing differences or similarities and detecting changes in circumstances or events. Identify body response variations, understand properties of medications and understand technical operating of equipment. Is knowledgeable and reacts within licensure standards / facility policies and procedures related to: medical emergencies, hazardous materials, infection control practices, accident care, interpersonal communication techniques, nursing practices, supervision of nursing assistants and licensed practical nurses, resident observation, sanitation practices, basic first aid principles, medication management, medical record documentation, medical terminology and nursing terminology. Monitors and reviews information from materials, events or the environment to detect or assess problems and identifies and intervenes in a solution. Develops constructive and cooperative working relationships with others and maintains them overtime. Organizes, plans and prioritize work by developing specific goals and plans to prioritize, organize and accomplish your work in an efficient and effective manner. Identify educational needs of coworkers, residents, family members, etc., and teach or instruct to improve the knowledge of others. Uses relevant information and independent judgment to determine whether events or processes comply with laws, regulations or standards and reports findings to your supervisor. Develop and build teams among coworkers by encouraging and building mutual trust, respect and cooperation among team members. Coordinate the work and activities of others by getting coworkers to work together to accomplish a task. Handling complaints, settling disputes and resolving grievances and conflicts of subordinate employees and notifying your supervisor when additional assistance is needed. Utilizing independent judgment, evaluate the skills of your licensed practical nurses and the needs of your residents and assign your subordinate employees to residents following this ongoing evaluation process.
