Patient Safety Program Manager

Lead and develop the entity-specific Patient Safety and High Reliability Organization (HRO) programs. Facilitate process improvements to reduce patient care errors and adverse outcomes. Analyze, categorize, and report adverse events data; use insights for continuous safety improvements. Coordinate standardized, data-driven patient safety initiatives and collaborate with operational teams. Lead investigations of safety issues using event analysis, Root Cause Analysis, FMEA, and other methodologies. Design and deliver educational programs on patient safety and HRO standards; mentor staff. Identify trends and areas for quality improvement; report findings to leadership and committees. Maintain expertise in High Reliability concepts and Just Culture algorithms.
