Job Description

We are seeking a dedicated and skilled nursing professional to join our team. This position primarily involves working in the Rehabilitation unit, with the flexibility to float to Medical/Surgical units as needed.

Essential Duties and Responsibilities

Support and promote the mission and values of the organization.
Provide professional nursing care to patients in the assigned unit and implement medical orders from the responsible healthcare provider.
Document comprehensive clinical information, including care plans, for each shift.
Request feedback on nursing practices and make necessary adjustments for improvement.
Communicate patient information effectively to ensure continuity and safety of care through bedside shift reports, multidisciplinary rounds, and hourly rounding using AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You).
Achieve unit-specific competencies and requirements.
Complete all required learning modules within established deadlines.
Promote a culture of safety and well-being, including efforts to minimize alarm fatigue.
Encourage a positive work atmosphere within the department each shift.
Ensure a safe and secure environment for patients, visitors, and staff by adhering to established protocols.
Identify and help resolve patient care safety issues.
Engage in a Just Culture environment by promoting error reporting and the adoption of safe practices.
Address issues through the appropriate chain of command.
Participate in daily unit-based safety huddles.
Stay informed about the organization's strategic goals and unit-based nursing-sensitive indicators (e.g., CAUTI, CLABSI, C. diff, HAPU, falls, hand hygiene, and VAE).
Actively participate in quality improvement initiatives to enhance patient outcomes.
Collaborate with colleagues across other departments to leverage expertise in various clinical specialties.
Support team members and assist in their development.
Set and implement performance goals based on self-assessment and performance feedback.
Ensure patient education materials are culturally relevant and accessible.
Serve as a preceptor for new nurses.
Prepare for DNV survey readiness.
Demonstrate practices aligned with shared governance principles and unit philosophy.
Contribute to problem-solving and conflict resolution within the team.
Delegate nursing care and unit responsibilities effectively to personnel.
Work collaboratively with nursing students and their instructors.
Engage in peer feedback processes during annual evaluations.
Participate in employee engagement surveys and activities.
Exhibit the core values and commitment to co-workers.
Manage the use of Earned Time Off (ETO) according to policy.
Complete required dysrhythmia training as part of orientation for telemetry-based departments.
Maintain current nursing licensure and relevant certifications as required for the department.
Perform other duties as assigned that are consistent with this role.

Job Requirements

Current New Hampshire RN license required.
Basic Life Support (BLS) certification is mandatory.
Graduation from an accredited nursing program is preferred.
Basic dysrhythmia understanding is preferred and must be obtained prior to orientation completion.
Crisis Prevention Institute (CPI) training is preferred and must be obtained prior to orientation completion.
