Monday to Friday, 8:00 AM - 4:30 PM - on-call/holiday rotations

Job Description

The Clinical Care Registered Nurse (CCRN) provides direct nursing care to participants, working independently while applying advanced nursing knowledge and skills to manage complex clinical situations. This role involves collaboration with both licensed and non-licensed personnel.

Responsibilities

• Triaging emergent situations and applying advanced nursing knowledge independently.
• Monitoring acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
• Utilizing care plans to provide clinic and home-based direct care.
• Administering treatments, procedures, and medications as prescribed to participants.
• Making regular home visits for care and treatment as ordered by the PCP, including wound care, lab draws, and nursing assessments.

Required Experience / Certifications / Licensure

• Bachelor's degree preferred in Science of Nursing; Associate's degree required.
• RN Nursing Diploma or Degree from an accredited School of Nursing or University.
• A minimum of one year of experience working with the frail, elderly, or long-term care population.
• Minimum of one year of home care experience preferred.
• One year of wound care experience preferred.
• Possess current CPR certification by AHA or willing to obtain.
• Must have reliable transportation and valid Michigan driver's license.
• Must be fully vaccinated or willing to be fully vaccinated against specified communicable diseases.
