Responsibilities Assessment of all recent admissions and acute issues of both subacute and long-term care patients Prevention of needless hospital transfers through early assessment and interventions Communication and collaboration with primary care and specialists Qualifications Previous RN experience - NEW NP GRADS WELCOMED! Ability to work as an interdisciplinary team member Good critical thinking/ assessment skills and the ability to function in a fast-paced environment Excellent written/verbal communication skills and computers Job Type Full-time Part-time Pay $120,000.00 per year (BASE SALARY for FT) PLUS, productivity bonus with the potential to earn up to $80,000.00 extra per year.
