Join Our Team as a Social Worker!

Green Hills Center for Rehabilitation and Healing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. In this role, you will play a vital part in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and enhancing overall quality of life within our skilled nursing environment.

Key Responsibilities:

• Provide emotional support and counseling to residents and their families
• Conduct psychosocial assessments and develop individualized care plans
• Coordinate discharge planning to ensure smooth transitions of care
• Advocate for residents’ rights and overall well-being
• Collaborate with nursing, therapy, and medical teams to support holistic care
• Assist residents and families with community resources and services
• Maintain accurate and timely documentation in accordance with regulatory standards
• Participate in care plan meetings and interdisciplinary team discussions

Qualifications:

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred
• Prior experience in a skilled nursing facility or healthcare setting is a plus
• Strong understanding of discharge planning and care coordination
• Excellent communication and interpersonal skills
• Ability to handle sensitive situations with professionalism and empathy
• Knowledge of state and federal regulations preferred
