Your Health Organization in Savannah is seeking a Clinical Coordinator to assist with all clinical needs for our patients. The role involves chart preparation, scheduling visits, and supporting providers and patients within a value-based care model. This full-time, 8-hour-per-day position (8:00 AM–5:00 PM, Monday–Friday) requires CMA or a licensed nurse, CPR certification, and strong communication skills to coordinate care across offices and facilities. #J-18808-Ljbffr
